Heparin Drip Titration on the NCLEX
Heparin drip titration: aPTT-driven adjustment, bolus, when to hold.
Heparin drip titration follows a nomogram based on aPTT. NCLEX tests recognition of the pattern.
aPTT target
1.5-2x control (typically 60-80 seconds).
Pattern
aPTT low → bolus + increase rate. aPTT therapeutic → no change. aPTT high → decrease rate. aPTT very high → hold infusion, recheck.
When to call provider
aPTT extremely prolonged (e.g., >150 sec). Bleeding. Sudden platelet drop (HIT).
Frequently asked
When recheck aPTT after adjustment?
6 hours after rate change. Time for new steady state.
Bolus when restarting?
Per nomogram, often yes if aPTT subtherapeutic.
