Sliding Scale Insulin on the NCLEX
When sliding scale works, when it doesn't, hospital protocols.
Sliding scale insulin is reactive, gives insulin based on glucose readings. Often inadequate alone.
How it works
Glucose checked at standard intervals. Insulin given per protocol based on level.
Limitations
Reactive, not proactive. Lags behind glucose excursions. Better as adjunct to basal-bolus.
When acceptable
Hospitalized T2DM with mild hyperglycemia. Adjunct to scheduled regimen.
Frequently asked
Sliding scale only in T1DM?
Inadequate. T1DM needs basal insulin to prevent DKA.
