WBC Differential on the NCLEX

Neutrophils, lymphocytes, eosinophils, basophils: what each tells you.

WBC count plus differential. NCLEX tests left shift, lymphocyte changes, eosinophilia.

Normal WBC

4-11 × 10^9/L.

Neutrophils (50-70%)

Bacterial infection raises. Left shift (bands, immature) suggests acute bacterial. Neutropenia (ANC <500): infection risk emergency.

Lymphocytes (20-40%)

Viral infections raise. CLL very high. HIV: low CD4 specifically.

Eosinophils (1-4%)

Allergies, parasites, drug reactions.

Basophils (<1%)

Rare elevations. CML.

Frequently asked
